This Privacy Notice describes how Long Term Care Solutions, Inc., and Long Term Care Insurance Services (collectively “LTC Solutions” or “we” or “our” or “us”) treats certain information collected or provided in connection with an end user’s (“you” or “your” or “user”) use of LTC Solutions’ websites, www.ltc-solutions.com, *.myltcguide.com, www.caresourcesolutions.com, and www.ltcenrollmentsolutions.com, and any other website, social media pages, or other digital medium owned or operated by LTC Solutions. This Privacy Notice also applies to any Personal Information collected, stored, or used by us, such as information you provide us through the above mediums, by email, mail, or telephonic communications (collectively, the “Site”).

The information provided in this Privacy Notice is designed to inform you of what information we collect, why we collect such information, how we use the information we collect, and the choices we offer, including how to access and update information. By accessing or using our Site, you consent to the Terms and Conditions and this Privacy Notice, including our collection, use, and disclosure of your Personal Information, as described below. This Privacy Notice is not a contract and does not create any legal rights.

Our Collection and Use of Personal Information

Personal Information is any information that can be used to identify you. This Privacy Notice does not cover data we, or our authorized third-party vendors or partners, collect that cannot be used to identify an individual or to pseudonymous data.

We get most Personal Information directly from you, through the Site, or from transactions with you. We might also obtain Personal Information about you from other Site users or other entities with whom we have contact. For example, we may obtain information through your employer. If you are sharing information with us about any other individual, you must obtain their consent.

Listed below are the categories of Personal Information we may collect that could pertain to you, and/or your spouse, and/or your family member(s):

  • Name, mailing address (work and personal), phone number (work and personal), email address (work and personal), social security number, birthplace, gender, date of birth, marital status, spouse’s gender, employer, employee ID number, date of hire, job title, height, weight, heath questions including medical history, previous diagnoses and medical treatments, primary care physicians contact information, medication usage and history, tobacco usage, beneficiary contact information, coverage applied for including additional riders and premium amount.
  • Information you provide when interacting with the Site, including transaction data and communications (email/DM).
  • Website analytics information, including when, how often, and for how long you use the Site; the content you submit and view; the name of the domain and host from which you access the Internet; and the IP address of the computer or device you are using.

We collect personal information to help identify you; to perform contractual obligations, including processing requests for insurance coverage; to communicate with you; provide customer service including responding to your requests for technical support, service information or other communication you initiate, to properly administer our Site; to research and analyze the use of our Site and services so we can improve our services; to notify you of promotions and services we think may interest you and for other marketing purposes; to detect and prevent fraud; and to comply with legal obligations.

 

We may combine your personal data with other data we obtain from our Services, other users, or third parties.  We may convert, or permit others to convert, your personal data into deidentified, anonymized, or aggregated data, as permitted by law.

 

You can choose to not provide certain information to us, but it may delay or prevent us from assisting you in obtaining insurance coverage.

 

 

Categories of Personal Information Disclosed or Sold

Listed below are the categories of Personal Information we disclose to third parties:

  • Your name.
  • Contact information, including your phone number, email address, and mailing address.
  • Information you provide to obtain a rate quote, including date of birth, employer name, marital status, gender of your spouse, and health history.
  • Information you provide in an application for individual or group LTC insurance coverage, including date of birth, social security number, and health information.
  • Information you provide when interacting with the Site, including transaction data and communications (email/DM).
  • Website analytics information, including when, how often, and for how long you use the Site; the content you submit and view; the name of the domain and host from which you access the Internet; and the IP address of the computer or device you are using.

Categories of Third Parties That May Receive Your Personal Information

In order to conduct our business and better serve you, we may disclose Personal Information and other information about you or your activities on our Site or while using our services with:

  • Third parties approved by you, such as insurance carriers and your employer.
  • Our service providers and vendors, such as website hosts, analytics providers, legal counsel, and various customer, tech, and operational support.
  • Law enforcement agencies, regulatory bodies, courts, or other public authorities, to comply with our legal and regulatory obligations, to respond to requests from governmental authorities, to protect our rights, to defend against legal claims, or as otherwise required by law.

Some third-party providers may independently collect Personal Information when you visit our Site. Our authorized third-party vendors, such as Cognito, Google, HubSpot, Microsoft, Citrix ShareFile, Jotform, Oracle OCI and SalesForce have their own privacy notices which govern the use, handling, and disclosure of your Personal Information.

We do not currently respond to Do Not Track browser settings because a uniform technological standard has not yet been developed. We reserve the right to update our practices and Privacy Notice as such standards are created and evolve.

Retention, Confidentiality, and Security of Personal Information

We store Personal Information for as long as is reasonably necessary to fulfill the purpose of collection or further processing as described in this Privacy Notice; as we determine is necessary for business records; and as required under applicable law.

We retain the following categories of information for the periods listed below:

  • Plan information, including name, address, email, phone number, plan design, policy number, premium, and effective date is retained for so long as the Policy is active with the Client. After the Policy/Client relationship closes, all such enrollee information is deleted roughly 120 days following end of Policy.
  • Recordings of agent assisted applications are retained for a period of ­­­­­six (6) years.
  • Website analytics information is retained for a period of one year.

We may also receive information from you, an insurance carrier, or your employer for which we have no business need. We make best efforts to promptly delete any such information.

We maintain reasonable physical, electronic, and procedural safeguards to guard your information. However, the Internet is not absolutely secure and, thus, we cannot promise guaranteed security. In the unlikely event that we believe that the security of your Personal Information in our possession or control may have been compromised, we may seek to notify you. As applicable, we will comply with notification requirements for a security breach and may also notify you by email.

Tracking Technologies

We may use tracking technologies, such as cookies, including tracking technologies managed by vendors or external parties. These tracking technologies can generally be categorized as follows:

  • Necessary or essential. These tracking technologies are used for core functionality, for example for security and to enforce your privacy preferences. These cookies are required for the full functionality of our Site.
  • Functional or analytics. These tracking technologies help to improve our services by allowing us to understand how our consumers engage with us and our Site.
  • Security.  To enable security features and allow us to detect activity that might violate our Terms and Conditions, such as unauthorized access or activity.
  • Location. We may use location-aware technologies to locate you, including as determined by your IP address, for purposes such as verifying your location and delivering or restricting content based on your location. Our Site may be personalized to provide you with more location-relevant content.
  • Advertising. These tracking technologies help us, and other parties personalize ads that are shown to you on our sites and services, as well as on other online services. If enabled, these tracking technologies will allow other parties engaged in interest-based advertising on our Site to understand your use of online services across platforms and over time.

We may combine tracking technology information with other personal data about you, such as combining your usage information with offline purchasing behaviors.

We or external parties may collect personal data about your online activities over time and across different online services when you use the Site.

Children Under 13

We do not knowingly collect information from minors and the Site is intended for adults over the age of 18. If you are a parent or guardian and believe that your child under the age of 13 has provided Personal Information to us without your consent, please notify us at privacy@ltc-solutions.com. If we become aware that information is or has been submitted by or collected from a minor under the age of 13, we will work to promptly delete this information.

Your California Rights & Transparency Notice

Please read below for the rights that apply to residents of California.

You have the right to opt-out of the sale of your personal information by LTC Solutions. As we do not sell your personal information, there is no need to submit a request to opt-out.

Additionally, you have the following rights:

  • You have the right to correct inaccuracies in your Personal Information in our possession.
  • You have the right to request we delete Personal Information we collected about you.
  • You have the right to request that we disclose the specific pieces of Personal Information we collected about you.
  • You have the right to request the categories of Personal Information we collected about you; the categories of Personal Information we sold or shared about you; the categories of third parties to whom the Personal Information was sold or shared, by category of Personal Information; the categories of Personal Information we disclosed about you for a business purpose; and the categories of third parties to whom we disclosed Personal Information for a business purpose.

If you or your authorized agent would like to submit a verifiable consumer request to correct inaccuracies, to delete, or for information, email us at privacy@ltc-solutions.com. You or your agent must provide your policy number and your employer’s name in order for us to verify your request.

You have the right to not be discriminated against for exercising any of these rights.

We do not sell your Personal Information to any third parties. We do not knowingly sell personal information of consumers under the age of 16.

Changes and Updates to the Privacy Notice

This Privacy Notice applies to all relevant information collected by or provided to us or our authorized third-party vendors or partners as of the Effective Date. When we make any material changes to this Privacy Notice, we will update the Effective Date and may inform you by email to the address associated with your account. We will treat your continued use of this Site or our services following such notice as your acceptance of the changes.
